The Community Health Improvement Plan (CHIP) was developed by a diverse group of organizations representing various sectors in Philadelphia, including public health, education, community development, health care, business, and social service. The CHIP was based, in part, on data from Philadelphia’s Community Health Assessment.
The CHIP identifies a set of priorities and strategies for improved health in Philadelphia, highlighting the role of non-governmental organizations. The three priorities of the CHIP are: access to care, behavioral health, and chronic disease related to poor diet and physical inactivity.
