COMMUNITY HEALTH NURSING SUPERVISOR
GENERAL DEFINITION
This is supervisory nursing work providing direction to a small group of nurses in a medical team and insuring the effective coordination of team activities with health care facility resources. Employees may supervise nurses or technical personnel working in specialized assignments, such as coordinating the delivery of managed health care services and reviewing and evaluating the quality and level of services; or specialized nursing work in the care and treatment of patients with a specific disease or condition. The employee is responsible for directing patient triage and patient case management and assigning these duties to subordinate nursing staff. Counseling and instructing patients in aspects of health care and in following physicians' orders is a significant aspect of the work. The employees work in a district health center or other City facility. An employee in this class functions under the supervision of a nursing superior and provides supervisory and nursing services in the implementation of an ambulatory health care program.

TYPICAL EXAMPLES OF WORK (ILLUSTRATIVE ONLY)
Assigns work to a small group of registered nurses and subordinate nursing personnel; supervises work by observation and evaluates work performance; confers with health care coordinators on problems encountered in the application of nursing practices and gives recommendations for their solution; trains and instructs nurses and subordinate nursing personnel in the administrative and clinical protocol to be followed in the health care program.
Resolves problems pertaining to patient flow in assigned unit; insures that patients are processed expeditiously and that facility resources are properly utilized; answers phone inquiries of a non-routine nature and interviews walk-in patients; supplies information to patients regarding program procedures and services; determines urgency for examination by physician for the patients and arranges for appointments.
Supervises the maintenance of records and medical treatments and related services as completed by subordinates; insures that adequate follow-up is performed for broken appointments and abnormal laboratory test results.
Takes patient history, including identifying data, previous medical, and hospitalization history, symptoms, and previous immunization and tests; reviews patient's records to insure accuracy and completeness; insures that results of laboratory tests are recorded; informs physician of any special problems requiring attention.
Prepares patients for medical examination and assists the doctor in examining and treating patients; prepares and gives medications to patients in accordance with doctor's prescriptions; obtains specimens for laboratory examination; charts and records the administration of medication and treatment and nursing diagnosis of patient's condition.
Reinforces physician's orders after examination to insure that patient adheres to the medication schedule; instructs patients of proper methods and procedures for administering medication and obtaining necessary specimens for laboratory examination; advises and counsels patients on the principles and techniques of preventive health care including diet and hygiene; makes referrals to other agencies for nursing, social and emotional problems.
Conducts group teaching in such areas as pre-natal and diabetic care.
Participates in team conferences and insures follow-up of decisions.
Provides nursing services according to standard orders for such services as administration of immunizations and well child examinations.
Specialized Nursing Assignments - Supervises nurses or technical personnel performing the above services for patients with a specific disease or condition or need; may report to an administrative superior and receive technical direction from a nursing or medical superior.
Managed Care Assignment - Supervises nurses engaged in the coordination of the provision of health care services associated with a variety of managed health care plans.
Performs related work as required.

EDUCATION
Graduation from an accredited school of nursing.
Completion of a comprehensive training program for the prevention of sexually transmitted diseases mandated by the Center for Disease Control if required by assignment
AND
EXPERIENCE
Three years of nursing experience.
NOTE:
A bachelor's degree with major course work in nursing may be substituted for one year of the general experience requirement.
PHYSICAL AND MEDICAL REQUIREMENTS
Ability to physically perform the duties and to work in the environmental conditions required of a position in this class.
LICENSES, REGISTRATIONS AND/OR CERTIFICATES
Eligibility for a license as a Registered Nurse issued by the Pennsylvania State Board of Nurse Examiners at the time of application and possession of such licensure at the time of appointment and during tenure of employment.
