HOMELESS SUPPORT SERVICES MANAGER

 

GENERAL DEFINITION

 

          This is management level resident care work directing, through subordinate supervisors, a staff providing a variety of social care services and overseeing the effective coordination of information with other units to ensure communication of participants’ needs.  The employee in this class is responsible for managing health care services for aged homeless adults and adults with mental and behavioral health issues.  Ensuring compliance with public health laws, planning and implementing social care services and coordination and contact with external housing agencies are a significant aspect of the work.  Work is performed under the supervision of an administrative superior.

TYPICAL EXAMPLES OF WORK (ILLUSTRATIVE ONLY)

 

Supervises social care supervisors and oversees daily activities of c subordinate health care personnel; supervises work and review of reports and evaluates work performance; confers with subordinate staff and superiors on problems encountered in the application of social care practices and gives recommendations for their solution; makes shift assignments and ensures minimum staff coverage; ensures that staff adhere to directives identified in the  homeless facility support plan while providing quality daily care in accordance with applicable laws and regulations.

Develops and establishes work policies; develops work procedures and routines and prepares written guidelines for staff; trains and instructs subordinate personnel in the administrative and clinical protocol to be followed in the program; trains staff to identify and address the challenges of providing services to older adults with  mental health, social, and physical  issues;  trains staff to recognize symptoms and side effects of psychotropic and pain management medications;

         

          Holds regular meetings to promulgate changes and analyze problems; interviews prospective employees and makes recommendations for hiring; participates in and/or directs the orientation and training of new employees and supervisors; meets with and discusses problems and work situations with union representatives:

          Analyzes participant and staff needs and activities to determine areas for service improvement or development; coordinates with social service, volunteer service and health service units for the provision of services; recommends the development and/or intensification of services for participants as needed; maintains computerized files; supervises the maintenance of records and medical treatments and related services as completed by subordinates; ensures that adequate follow-up is performed; ensures that participants are not engaging in any activities that may cause harm to themselves or others; .Coordinates the activities of case managers, site directors and departmental staff with care strategies to ensure housing.

          develops and ensures enforcement of supply dispensing procedures ensures compliance with state, federal and local health and safety laws, regulations and guidelines concerning  care.

          Prepares reports on unit activities;; reviews daily log of condition of the facility and reports of any repairs or issues that need attention; reviews frailty and wellness reports on participants; meets with and advises staff on issues and behavioral problems of participants whom require higher level of care; investigates complaints of injury and health referrals; reports plans of correction directly to PA Department of Public Welfare.

          Performs related work as required.

 

REQUIRED KNOWLEDGE, SKILLS AND ABILITIES

 

KNOWLEDGE OF:

 

•         the principles and techniques of personal and nursing care

•        

•         federal, state and local health and safety laws, regulations and guidelines concerning nursing home and personal care facilities

•        

•         clinical guidelines and their practical application in the provision of adult personal care

•         principles and techniques of staff development and sensitization to the emotional and social needs of participants

•         administrative principles and procedures for health service programs

•         supervisory methods and techniques

 

SKILL IN:

 

•         Microsoft Word and Microsoft Powerpoint

•         managing the coordination of health of participants

 

ABILITY TO:

 

•         direct and train a staff of supervisors and aides providing homeless support care

•         learn and apply codes and regulations governing the provision of personal care services

•         maintain records and prepare comprehensive reports

•         direct the training of employees in health care work

•         successfully complete yearly training required by the Commonwealth of Pennsylvania

·                  direct the coordination of the provision of a variety of participant care services associated with participant support plans

·                  evaluate and resolve problems related to the administration of homeless services

·                  establish and maintain effective working relationships with participants, staff ,associates and administrators

 

MINIMUM ACCEPTABLE TRAINING AND EXPERIENCE  (The following statement represents the minimum training and experience standards which will be used to admit or reject applicants for tests.  Applications submitted by candidates for this class will be reviewed based on training and experience requirements as approved on 1/25.)

 

EDUCATION:

 

          Graduation from an accredited school of nursing.

 

AND

 

          Currently licensed as a Registered Nurse by the State Board of Nursing for the Commonwealth of Pennsylvania. 

 

AND

GENERAL EXPERIENCE:

          Three years of experience providing resident care services in a hospital, nursing home, assisted living facility, or personal care home.

 

AND

SPECIFIC EXPERIENCE:

 

          Two years of experience supervising direct care staff in a hospital, nursing home, assisted living facility, or personal care home.

 

OR

 

          Any equivalent combination of education and experience determined to be acceptable by the Office of Human Resources which has included current licensure as a Registered Nurse by the State Board of Nursing for the Commonwealth of Pennsylvania and the specific experience.

 

PHYSICAL AND MEDICAL REQUIREMENTS

 

          Ability to physically perform the duties and to work in the environmental conditions required of a position in this class.

 

LICENSES, REGISTRATIONS, AND/OR CERTIFICATIONS

 

          Possession of a proper class motor vehicle operator’s license as issued by the Commonwealth of Pennsylvania at the time of appointment to the Resident Care Services Manager class.
