This is health care work supervising social care services rendered to adult and elderly participants on an assigned shift in a homeless services facility. Employees are responsible for the placement of shelter residents into nursing or personal care facility. Work is performed under the supervision of a higher level supervisor. Employees participate with others in establishing sustainable and accessible housing. Work may require light physical exertion.

TYPICAL EXAMPLES OF WORK (ILLUSTRATIVE ONLY)
Assigns work to a group of aides; may inspect personal care facilities and nursing homes to see that participants are properly cared for; routinely inspects shelters, transitional housing and other locations to make sure participants needs are met.
Directs aides in proper methods and techniques of measuring vitals submitting documentation for elderly participants and determining appropriate care; handling difficult individuals in need of supportive housing; conducts in-service training classes; establishes and maintains relationships with external stakeholders for placement of elderly individuals; monitors participants to ensure housing is sustained.
Schedules subordinates' daily work assignments; assigns subordinates to various locations within the city to ensure adequate coverage in all areas; approves overtime to maintain necessary coverage; approves requests for location changes, as appropriate.
Participates in establishing housing for individuals as a member of a team providing social services.
Attends in-service training classes as assigned.
Maintains records and prepares reports.
Performs related work as required.
KNOWLEDGE OF:

MINIMUM ACCEPTABLE TRAINING AND EXPERIENCE (The following statement represents the minimum training and experience standards which will be used to admit or reject applicants for tests. Applications submitted by candidates for this class will be reviewed based on training and experience requirements as approved on 1/25.)
EDUCATION:
Equivalent to completion of the twelfth school grade.
EXPERIENCE:
One year of experience providing general health and support care for adults and elderly participants within a shelter or transitional housing facility.
Or any equivalent combination of education and experience determined to be acceptable by the Office of Human Resources.
Ability to physically perform the duties and to work in the environmental conditions required of a position in this class.
Possession of a valid proper class motor vehicle operator's license as issued by the Commonwealth of Pennsylvania prior to appointment and during tenure of employment as a Social Program Services Supervisor, if required by work assignment.
Certification of attendance at an approved school of nursing aides and eligibility for state registration, if required by assignment.
