HOMELESS SERVICES SUPPORT AIDE

GENERAL DEFINITION
This is general health care work providing support care in a homeless services facility. An employee in this class performs routine nursing support and clerical tasks. Work includes weighing and measuring participants, taking blood pressure, and keeping vital signs equipment and assessment rooms clean and neat.
The work has some disagreeable aspects. Light physical effort is required. Work is performed under the supervision of a higher level medical superior.

• The position must be located in the Office of Homeless Services.

BOTH SPECIALTIES
Performs routine nursing duties such as weighing and measuring participants, and taking and recording temperature, blood pressure, pulse and respiration; attends and cares for adult and elderly participants; provides medication reminders; observes precautions to prevent participants from injuring themselves; communicate with participants during activities; conforms and attends to their personal needs; participates in the rehabilitative programs of the facility by reinforcing new behaviors learned by participants; may monitor the activities of participants to ensure their safety and compliance with rules and regulations.
Administers frailty questionnaire utilizing Microsoft Office Suite to assist in the collection of contact information and to gather baseline summaries of participants' medical and social needs; assists participants in verifying insurance coverage and in coordinating and scheduling transportation and appointments; may communicate with Philadelphia Corporation for Aging (PCA) workers and assist participants in reaching out to doctors' offices, and necessary entities to acquire required documents for participation in social service programs; may monitor and enter information from External Agency Referrals (EAR)s into Office of Human Services HMIS system.
Cleans and sterilizes equipment and supplies; may clean, dust and spot mop rooms; prepares lists and orders supplies as needed; may gather and deposit soiled laundry items; distributes fresh supplies.
May transport equipment and/or participants to various locations when requested.
Performs related work as required.
Performs the duties described below with members of a designated non-English language speaking community.
REQUIRED KNOWLEDGE, SKILLS AND ABILITIES
KNOWLEDGE OF:
• the English language and spelling
• principles of healthcare
• compliance with privacy requirements in a social services facility

• do routine housekeeping and maintain cleanliness of clinic rooms and area
• establish and maintain effective relationships with participants, other social program employees, and the general public

MINIMUM ACCEPTABLE TRAINING AND EXPERIENCE (The following statement represents the minimum training and experience standards which will be used to admit or reject applicants for tests. Applications submitted by candidates for this class will be reviewed based on training and experience requirements as approved on 1/25.)
BOTH SPECIALTIES
EDUCATION:
Education equivalent to the completion of the twelfth school grade.
AND
EXPERIENCE:
Six months of paid experience in the care or treatment of geriatric or physically ill patients or residents in a healthcare facility.
NOTE: Actively registered with the Commonwealth of Pennsylvania Nurse Adie Registry meets all experience requirements.
AND
Sufficient training and/or experience to be able to understand and speak fluently and clearly in English and a designated non-English language.
PHYSICAL AND MEDICAL REQUIREMENTS
Ability to physically perform the duties and to work in the environmental conditions required of a position in this class.
LICENSES, REGISTRATIONS, AND/OR CERTIFICATES
Possession of a valid proper class motor vehicle operator's license as issued by the Commonwealth of Pennsylvania prior to appointment and during tenure of employment as a Health Care Aide if required by work assignment.
Actively registered and in good standing with the Commonwealth of Pennsylvania Nurse Aide Registry prior to appointment and during tenure of employment as a Health Care Aide if required by work assignment.
